My right testicle is swollen?

my right testicle is swollen since 4-5 months! its almost twice the size of the other..it usually doesnt pain but once in a month it pains very badly at night time when i go to sleep!i even can find a seed above the testicles!it is wer d pain is(once in a month)the pain last for a week or so??is it hernia??
